Informed Consent and Counselling Agreement
Counselling Services
Individual counselling is provided primarily online via Zoom. Sessions are 90 minutes at $150 per session.
Confidentiality
Counselling is confidential except where disclosure is required or authorised by law, including circumstances involving serious threats to health or safety. Information may be shared with other professionals with client consent. De-identified information may be discussed in professional supervision.
Privacy and Records
Personal information, including contact details, intake information and counselling notes, is collected to provide counselling and meet professional and legal obligations. Clinical records are stored on a password-protected laptop with restricted access and retained for at least seven years after the final appointment, or longer where required by law.
YouCanBookMe manages bookings and processes information in the United States. Zoom delivers online counselling and may process personal information internationally.
Clients may request access to or correction of their records and raise privacy concerns with the counsellor or the Office of the Australian Information Commissioner.
Cancellation Policy
Cancellations or rescheduling require at least 24 hours' notice. Late cancellations and missed appointments incur the full $150 fee.
Client Rights
Counselling is voluntary. Clients may ask questions, decline to discuss matters or discontinue counselling. Online sessions should be attended from a private location. Sessions will not be recorded without consent. It is the Client’s responsibility to inform of disengagement from service.
Informed Consent
